Overheight Load Safety Checklist Please check off the box for each item as you go over it with your escort. Please return the completed checklist with your paperwork. [ ] IF THE ESCORT’S POLE HITS AN OVERHEAD OBSTRUCTION, YOU MUST STOP! [ ] The driver and escort have jointly measured the height of the load. The pole has been set 6 inches higher than the load. The driver has verified the quality of the escort’s pole. [ ] The loaded height has been called into the dispatching office. [ ] The escort has been given a copy of the permit. The driver and escort have reviewed the routes of movement and any trouble spots. The driver and escort have reviewed special notations and restrictions listed on the permit. The driver and escort will continue to review the routes at each stop. [ ] The driver has reviewed the hardcopy to verify whether any surveys were done. If so, the driver must verify he received the survey notes. The driver and escort must review the survey notes with special attention to problem areas, or bridges with lane specification for clearances. [ ] Any disagreement between the driver and escort as to the routes to follow, or any other aspect of the move, shall be resolved by calling the terminal assigning the escort or the Carlisle permit office. [ ] The driver and escort are to observe all permit rules and regulations, including such items as speed limits, hours of travel, etc. [ ] No movement is to be made through highly congested areas during peak commuting times. [ ] One hour after commencing movement and periodically thereafter, the escort will stop and verify the height of the pole and the height of the load. [ ] The driver and escort have CB radios in good working order. The load will not move unless they do. [ ] The contractor and escort must agree on a CB channel that allows effective communication. Under no circumstances is channel 19 to be used. [ ] The driver and escort must maintain a minimum of approximately ¼ mile (1,320 feet) distance between vehicles to allow for proper communication and safe stops. [ ] The escort must communicate with the driver as each overhead obstruction is cleared by signifying “OK”, etc. [ ] If the escort’s pole touches an overheight obstruction, the escort must immediately notify the driver by stating, “I hit, shut it down.” Both vehicles must pull over onto the shoulder. Following consultation, the driver can proceed up to the obstruction at which time the clearance will be measured. Unless the driver and escort are satisfied that the load will clear, no further attempt to “ease” under the obstruction is to be made without first calling the Carlisle permit office. Alternate routing may be required. [ ] IF THE ESCORT’S POLE HITS ANY OVERHEAD OBSTRUCTION, YOU MUST STOP THE TRUCK! DO NOT TRY TO EASE UNDER THE BRIDGE. 5/08
